Birth Records are not public information. Copies of birth certificates are only available to the person named on the certificate (18 years or older), the person’s parents, a lawful representative, or anyone who presents a court order. If you were born within the village limits of Johnson City, N.Y., and are over 18 years of age, you may obtain a certified copy of your birth certificate from our office.

You may obtain a copy of a Birth Record either in person or by mail. Download this application (Click Here). Print the form and follow the instructions below to mail the application or bring the completed form in person to the address below.

In Person Drop Off:

  • Bring the completed application
  • Photo identification of yourself
    • Driver’s License
    • Non-driver’s license, Passport, Naturalization Papers, Military ID, Employer’s Photo ID,
    • Two utility bills showing applicant’s name & address AND a Police report of lost or stolen ID
  • $10 fee (cash, personal check, certified check, or money order) for each record.

By Mail – Send the Following:

  • Remit the completed application
  • A photocopy of one of the following forms of identification
    • Driver’s License
    • Non-driver’s license, Passport, Naturalization Papers, Military ID, Employer’s Photo ID,
    • Two utility bills showing applicant’s name & address AND a Police report of lost or stolen ID
  • Remit the required fee of $10 per certificate in the form of a check, money order or certified check only, made payable to “The Village of Johnson City, NY”. DO NOT send cash or stamps.
  • Mail to: Office of the Village Clerk, 60 Lester Ave, Johnson City, N.Y. 13790

Death Records are considered confidential by New York State Law and are available to the spouse, parent, or adult child of the decedent; someone who can demonstrate a legal claim; Veterans Administration Hospitals; federal agencies for official purposes; New York State agencies for official purposes; and someone who can demonstrate an official medical need. If the decedent died in the Village of Johnson City, N.Y., the record is on file in our office.

Genealogical Research

Uncertified copies or abstracts from records of birth, death and marriage may be provided for genealogical research purposes subject to the restrictions specified in this summary. All requests must be submitted in writing and include payment of the applicable statutory fee. The applicant shall be required to pay the specified fee for the time spent for the search and uncertified copy of notification of no record.

What records are available?

  1. No Information shall be released from a record of birth which has been placed in a confidential file pursuant to Public Health Law Section 4138.
  2. No information shall be released from a record of birth unless the record has been on file for at least 75 years and the person to whom the record relates is known to the applicant to be deceased.
  3. No information shall be released from a record of death unless the record has been on file for at least 50 years.
  4. No information shall be released from a record of marriage unless the record has been on file for at least 50 years and the parties to the marriage are known to the applicant to be deceased.
  5. The time specified in b, c and d are waived if the applicant is a descendant or has been designated to act on behalf of the person whose record is being requested. A descendant is a person in the direct line of descent. The applicant shall provide documentation of descendancy prior to the release of information in those instances where a waiver of the waiting period is requested. A party acting on behalf of a descendant shall further provide documentation that the descendant authorized the party to make such application.
  6. All uncertified copies, abstracts, or information issued for genealogical research purposes shall be clearly marked with the statement “For Genealogical Purposes Only”.

The Town of Union Clerk’s Office issues the marriage licenses for those couples to be married within the Village of Johnson City.
